Thank you for completing our COVID 19 questionnaire. We look forward to seeing you at your appointment. Please note our new office policies:

• We ask you arrive 5 minutes prior to your appointment in order to complete COVID 19 forms
• We ask that you wear a face covering or mask to your appointment
• We will be taking a forehead temperature before you enter the office
• We are not allowing anyone to wait in our waiting room at this time.
      *Exception if patient is a child accompanied by ONE adult.
• An additional $10.00 personal protective equipment (PPE) will be charged per appointment, this charge will not be covered by insurance.
